Continuing Care

Longer-term check-ins, peer connection, relapse-prevention refreshers, and referral back to treatment when needed for clients, families, and referral partners.

In Development

Continuing care supports the period after formal treatment intensity decreases. It helps keep recovery connected to accountability, community, and a clear route back to treatment if the person needs more support again.

Intensity: Long-term follow-up and re-entry support | Housing: Coordinated separately | Clinical: Partner / pending

Potential fit

  • Clients stepping down from structured treatment.
  • Alumni who benefit from ongoing connection.
  • Families and professionals planning long-term recovery support.

Planned scope

  • Peer connection
  • Recovery check-ins
  • Alumni meetings
  • Community events
  • Mentorship opportunities when operationally available
  • Relapse-prevention refreshers
  • Referral back to treatment when needed
  • Long-term recovery planning

How it works

  1. 01

Continuing care does not promise lifetime access; it reflects what is operationally available.

  2. 02

Long-term support can include peer, alumni, and treatment re-entry pathways.

  3. 03

This service often overlaps with outpatient follow-up, recovery housing transitions, and community-based support.
